I look at it and the phrase "I wear my heart on sleeve" comes to mind. Maybe that wasn't the idea, but that's kinda what I see here.

I agree with the others, lactation comes to mind... especially males (I should know)... If that is the purpose, it is achieved. Otherwise, I think this is a 2 (no buy).

you could make the text like a neon sign, that should eliminate your color issue. And PLEASE don't put it on a pink shirt, even if the heart would be pink on a black shirt.

nah, i see it as vampire-y....they wouldn't have to go therough the trouble of biting your neck, just suck the blood straight through the straw. but i think it would work better in different colors...or maybe with a more realistic heart.
