Only just make the design a little smaller, about 90%.
What I like, is how I am totally not disgusted by the puke, probably because its got rainbow colors.
I must be interpreting the clown being dressed as one, and though we all know he's a human like everyone else, by nature he suppose to puke whatever he ate which is supposed to be disgusting, but then its funny, as if it happened just because he's a clown.

This is the first shirt I've seen that actually got me to laugh out loud. Love the idea, love the design, and I really love the sparkles. I'm definately not loving the yellow, though. Put it on any other color, just please not yellow.

I would buy it only if it wasn't a yellow shirt, but I see you used yellow as the same as the middle rainbow color, so if you make the shirt Natural color and then remove everything white in there, you'll have 4 colors to admit to.
