Yes, possibly lose the back design, maybe use the firetruck and the hat?

I like the fireman approach, but a doctor seems to make more sense at a glance.

Then again, representing a doctor while keeping a good design might be tough...

the shadows are quite large and distracting. i really don't see how they are needed here. if you want to convey that they are standing on a ground of some sort sometimes just a inconspicuous horizontal line behind them does the trick.
