i love this illustration! the only think is that the title makes it, without it its a little hard to get....but all the med signs and the seattle back drop helps. awesome job!

Even without the title, If you look long enough, one of the cows has a name tag that says 'mc dreamy'.

I just had a thought: Maybe name the barnspital "Seattle Graze" after the show's "Seattle Grace". Though it's so far in the distance you probably couldn't read it.

You don't have to get the 'joke/pun,' it is a fantastic shirt on its own. Who doesn't think cows in scrubs are funny.
