Why would you want to advertise that you 'want help'.

say you got better in a month since buying the shirt, you wouldn't want to continue wearing it because you would remember where you once were. I think a limited amount of people would actually wear this, let alone continue wearing it. Only because the design conveys somewhat of a serious message.



I think it could be done in a way that wasn't so aggressive. Perhaps something which said "I need help" but designed in a way that was fun, which gave the message that the wearer was someone who was fun, can take a joke and is ok that they're are slightly crazy in a good way.
