Definitely, the monkey doc alone would be awesome. Or the doc with the small pill. The big one, I'm not sure why it's there...

The big pill definitely shouldn't be in there, but you were right in thinking there should be something in that area... maybe just a smiling patient?
Also, would get rid of the white backdrop if it's printed on a lighter color like yellow |

About my design
if you're afraid even scared of drinkin up your anti-biotics.. try this one..
i used to put this design over BURGANDY, CHOCOLATE and BANANA YELLOW. but then this Design is also applicable to other tee-color.
