I think the "hearing voices" idea would be more evident if the guy looked more distressed about it. his hair should be messed up (schizophrenics typically have bad hygiene) and his head should be down, maybe with his hands on the sides of his head and his eyes wide open. I'm not sure exactly how to change his expression, but he looks too calm, rather than mad.
