I don't think the text is necessary at all. Maybe the orange could be in the doctor's other hand, lower than the apple. I think people will see the doctor and apple and that phrase will automatically pop into their heads. :)

Yeah, I like the placement lower on the shirt, but maybe you could take the doctors lefthand and wrap it around the side of the shirt so he's still holding the orange.
