several times. although i've never personally made a point of going up to a person to tell them i've designed it. i tend to hear more from friends who seem to see quite a few of mine about. they have no qualms about approaching people about it which is always funny to hear. one even chatted up a bloke who was wearing one of my designs, using it as an opening line - "hey my friend designed that..."... they "rocked out" together in the end. Hah.

I played basketball a couple of months ago, just in a social league, and one of the guys inn the other team was wearing my Uprising design. First time i've seen one of my own out there.

A couple times I've seen people wearing "I Listen To Bands That Don't Even Exist Yet" at various concerts I've been to. Also seen someone wearing "Historical Superheroes" and "Sees The Day" at craft fairs I've done for Made With Awesome. It's also really awesome when you see someone else's Threadless design that you're buds with.
