The title pretty much sums it up. I'm curious if the limit is 4 colors PLUS the background color of the shirt, or 4 colors INCLUDING the shirt.

Nope, it means 4 unique colors in the design being printed. That doesn't include the wide variety of canvas colors it could be printed on.
