I really don't know what needs to be changed here. Maybe make the image a bit smaller on the chest. Maybe redo the guy's ears, they seem overly complex. but awesome job, nonetheless

the first thing you should change is the title, because it's grammatically incorrect. it should say "i don't feel so well." and maybe you should try putting it on a different colored shirt, to balance all of the bright colors out.
