I'd change the perspective on the hands (sideways like that, they don't really look like hands.) Also, your design can't be printed on the sleeves; read the submissions guidelines, esp. the 'reasons for rejection' PDF.

Would love to see this on a real tee. You should really redo the hands and make left and right unique instead of just mirroring them. If you want it to look like a doctors' hands, you should consider changing the shirt color to hospital-blue and the gloves to white or the other way around.

But in the end: who would buy this? Surely not guys and I think girls neither...

i like the colors and how they blend but the hands are drawn really bad , you should use a reference and i also think you should work on the fade of the arm maybe position them somehow different

I would suggest rotating the hands so you see a bit more of the fingers.
