im not sure what program you're using but it'd be nice to see the image with more crisp lines, like less wobbly. Thats just my opinion though, its a great idea! And it might be cool if you had an image in the center of each card like real cards do.

Love the puppy as the sympathy card! -- maybe have the cards in a color separate from the shirt?

I like white cards a whole lot more. And as G.O.B. pointed out previously, the design could use some finer lines.
I'm not sure about what the message you're trying to portray is trying to say; maybe i'm just dumb or over-analyzing it, but everyone else seems to be getting it, so I'll just roll with it!

you should work more on the icons , they look a little to rough with messy lines , maybe vectorize em and be careful
also
if you have some time I would be grateful if you scored my design octo shirt not to have so many white spaces in the colored space , and maybe the cards should be white on gray shirt

I personally like the hand drawn quality. I do agree that white in the cards would help. I also think the universal icons are the best ones: Race and Sympathy. The Foreign could be a globe, the Female could be Gender with both Male and Female symbols, and Religion could include another Religious symbol or two. These are my suggestions to make the design appeal to a wider audience. Good start and good luck.
