Very interesting design. I don't know that they'll be able to print over the shoulder like that, but I'm liking the design.

I think you should work an Everlast-ish logo in somehow and drop the text.

the brains look cool. think you could work on the stitches… and i don't really get the concept.

What if you made the brains look more stitched, or like the wrinkles are individual fuzzy strands of wool... so they have the texture of wool mittens

I like it, but the brains are a little too shiny. I would stylize them more. make them more matte.

What I like: the idea, and the brains. What I don't like: the blue stitches/cord, maybe a more realistic cord could help. I don't know about the text. Also, you could try adding a subtle shadow underneath the brains (and even the cord, why not). Anyway, keep it up, I think you're on the right path.
